1. The Death Certificate (Model 3C)
Completed and signed by the doctor who certified the death.
If you don't have it, contact 112 to have a doctor come.
2. The Cremation Request (Model II)
Required only in case of cremation. Completed by a second guard doctor of the municipality.
3. Civil Identity Documents
The identity card of the deceased, their driving license and, if applicable, their official marriage certificate.

As a philanthropic ASBL, we reduce costs as much as possible:
- Free hearse and flower delivery van.
- Vehicles invoiced only at fuel cost price (+30 € maintenance).
- Free funeral equipment (cooling tables, trestles, stretchers).
- Free floral decoration graciously loaned (silk flowers worth €3,000).
